Electronic Form Submission Guidelines
- Please complete all required fields in the form accurately.
- Enter your active personal email address. This is required to ensure the PDF copy of the completed form is delivered successfully.
- Download the form file from your personal email and print it.
- Complete the original handwritten signatures, then submit the printed form to the college email coordinator to forward it to the Website Unit at the University Presidency for approval.
- After receiving your university email account, the beneficiary must change the password.
